Attached please find the Photograph Information and Release Form to be signed by members and their parents, Advisory Board members, and other individuals associated with your assembly who may be included in pictures taken at Rainbow functions.

It has come to our attention that it is best for our organization, and all concerned, that documentation of permission to use photographs should be on file.  The form provides information on Rainbow polices for the use of pictures.

Also attached is a Photograph Release List for you to keep a record of the names of those who have signed releases or declined the opportunity to grant permission.  

• Please obtain signatures on the release forms
• Record names on the assembly list and keep a Xerox copy with assembly records
• Optional: make copies of individual forms for assembly records
• Mail in completed forms and the original Assembly list as soon as possible
• Update information as needed

Pictures are a vital part of promoting the organization, so having permission to utilize the photos is extremely important.  As always, we encourage submission of photographs for promotional purposes to the Illinois Grand Jurisdiction and local assembly webpages, Illinois Rainbow News, local scrapbooks, and newspapers.  When submitting photos for any of these purposes, please make sure that everyone in the photographs has granted permission to use their pictures via the photograph release form.
